The image below was sent to me by a reader who went for a visit to a physician, and was told that she could not be seen unless she filled out this race and ethnicity questionnaire, and put her name on the form. She initially refused, but they insisted that it was required or she could not see the doctor. The form does indicate that it is mandatory under Section 4302 of Obamacare. Qualifications for healthcare should be used on income based questions alone.I´ve been wondering about this since this started if they asked the race but never heard that it did.Bottom line if you´re white don´t waste your time. In the left´s eyes you have a rich uncle or someone that could pay yours.How often does the left rant about profiling?That´s exactly what this is.

Add a box to the race list and check it. Label it as "Human".

Add a box to the ethnicity list and check it. Label it as Texan (for all of us Texans).

I stand with #1. Nowhere on any form is Scots-Irish mentioned! Then, on a form at the osteopath´s office I was offered the choice of marking Jehovah´s Witness on a form. I carped aloud that Presbyterians were getting short shrift!

I could have marked the box ´´decline´´ but opted for truth instead. That will set me apart, no doubt.

Read all statements presented for your agreement carefully, even if you´re not using Obamacare. Yes, I know they take a long time to read--do it! As I read a long set of statements at the PCP´s office I saw that, basically, it gave the medical staff permission to do anything they deemed necessary for my medical care. Like withhold certain treatments because of my age? Like do not resuscitate because they think it´s the right thing for me? Like treatments and prescription drugs I don´t want? So I wrote a sentence above my signature saying I am to be consulted about any and all treatment before any decision is made.

Btw, when I questioned the statement the front desk person shrugged and said you have to sign it or we can´t treat you--and added their lawyers composed the statement.

I´m with poster 16.
On my initial visit to a specialist I was handed the usual papers to fill out and sign.

Among the papers was a release for the doctor to check my credit rating. This despite my proof of insurance.

Whenm I bristled the person at the window told me many people object and ´don´t bother with it´. I wrote declined on the paper and handed it in.

I noticed what 6 did: La Raza is listed under ethnicity. I thought that was a political org. It means THE RACE, as in The Master Race. Where have we heard that before? We should demand the govt. institute a Don´t Ask-Don´t Tell policy re: race and ethnicity. In the interest of fairness, of course.

This form was clearly created by an uninformed person, I doubt it is a federal or state form. If it was created by the doctor´s office I would reconsider my choice of doctors.

I will continue to do so. It is a defensive move against the intrusive information gathering that is totally out of hand.
I have been doing this for decades.
No one has my correct BD,except the goverment, which you cannot get around, but anyone with no legal reason to have it, gets a fake BD too.

To properly answer the questions, you must first determine eggzackly what you are.

You must check with The United States Bureau of Racial Classification.

Does one drop of black blood qualify for race preference? Where is that list of Official Hispanic Surnames? What if you are a Mexican named Schwartz, which some are??

# 16 and #18 are spot on. Some new patient forms have a stamped paragraph that says you are agreeing to let the physician do "anything" I mark through it and initial it. Another specialist had a power of attorney agreement and if there was a lawsuit, the checks went to him, along with cars etc. Another offered a living will decision.

If you sign these forms without reading them, shame on you. I am not above getting up and walking out.
